What is the SSS E1 Form?

The SSS E1 form — officially the Personal Record (issued together with the UMID Card Application as the E-1/E-6) — is the record created when a person registers with the SSS for the first time and is issued an SS Number. It captures your personal details, address, and declared beneficiaries.

Since 15 July 2020, SSS registration is done online. You apply for your SS Number at sss.gov.ph, and the E-1/E-6 Personal Record is generated automatically from your application — there is no need to fill out a blank form by hand. This applies to anyone registering for the first time, whether you will be an employee, self-employed, an OFW, or a non-working spouse.

Who Registers for an SS Number?

Your employer does not obtain your SS Number for you. Once you are hired, the employer reports you for coverage and remits your contributions — but the SS Number is yours and you apply for it.

How to Register — Step by Step

Step What to Do
1Go to sss.gov.ph/become-an-sss-member/, open “Apply for an SS Number online,” read the procedure, and click “Start”
2Complete the Registrant Record Verification, certify your information is correct, and submit. SSS emails you a link to continue — it is valid for 5 calendar days only. If it expires, submit a new application.
3Open the emailed link and supply your basic, contact, address, social status, place of birth, and beneficiary details, then choose your purpose of registration
4Review your entries and click “Generate SS Number.” Your SS Number is issued immediately. You can print or download the Personal Record (E-1/E-6) form, SS Number Slip, and Transaction Number Slip — these are also emailed to you.
5Submit your supporting documents — upload them online during the application, or present them at an SSS branch — so your SS Number can be tagged “Permanent.” A separate emailed link (valid 30 calendar days) lets you create your My.SSS online account.

Valid IDs and Documents for SS Number Issuance

The main document for SS Number issuance is your PSA-issued Birth Certificate. If you do not have it, you may present any one of the alternative documents below. If you have none of those, you may instead present any two secondary documents — both showing your correct name and at least one showing your date of birth. IDs must be current; expired IDs are accepted only for retirement, disability, death, funeral, and pension transactions.

  • PSA-issued Birth Certificate (the main document)
  • Baptismal Certificate
  • Driver’s License
  • Passport
  • PhilSys ID / ePhilID (Philippine National ID)
  • Professional Regulation Commission (PRC) card
  • SSS Digitized ID
  • Seaman’s Book (Seafarer’s Identification and Record Book)
  • UMID Card

If you have none of the above — any two of these

  • PhilHealth ID card or Member Data Record
  • Pag-IBIG Member’s Data Form or Transaction Card
  • Voter’s ID card, or Affidavit/Certificate of Registration
  • NBI Clearance
  • Police Clearance
  • Postal ID card
  • School ID card
  • Senior Citizen card
  • Taxpayer’s Identification Number (TIN) card
  • Company ID card, or ID issued by an LGU (barangay, municipality, or city)

Frequently Asked Questions

Can I apply for an SS Number if I’m over 60?

SSS membership is built around contributions paid toward future benefits, so there is an age limit for first-time registration as a contributing member. If you are at or near 60 and have never been an SSS member, confirm your eligibility directly with SSS before registering, as your options depend on your situation. If you are under 60 and not yet a member, register as soon as possible — the earlier you start contributing, the higher your future benefits.

I already have an SS Number from a previous employer. Do I need to register again?

No — your SS Number is a lifetime number and you should never apply for a second one, as having more than one SS Number delays the processing of future claims. If you are changing membership type to self-employed, submit a Member Data Change Request Form (SSS Form E-4) to an SSS branch. If you forgot your SS Number, request verification of your existing number at the nearest SSS office, or check your My.SSS account if you already have one.

What does Temporary vs Permanent membership status mean?

When your SS Number is first generated online, it is tagged Temporary, which can be used for contribution payments only. To claim any SSS benefit or apply for a loan, your SS Number must be tagged Permanent — this requires submitting your approved supporting documents, either uploaded online during your application or presented at an SSS branch. Complete this step promptly.
