What is the SSS E1 Form?
The SSS E1 form — officially called the Personal Record form — is the membership registration form for new SSS members who are not covered through an employer. If you are a private sector employee, your employer registers you directly and you do not need to fill out an E1 form. If you are self-employed, an OFW, or a non-working spouse applying for the first time, you use the E1 form to register and get your SS Number.
SSS has moved most of the registration process online. You now apply for your SS Number at sss.gov.ph first, print the generated form, and then bring your supporting documents to the nearest SSS branch to complete registration. The E1 form is used as the physical record submitted to the branch.

How to Register — Step by Step

Supporting Documents Required
Bring the original or certified true copy of at least one primary document to the SSS branch. If you do not have a primary document, bring any two secondary documents, both showing your correct name and at least one with your date of birth.

Frequently Asked Questions
Can I apply for an SS Number if I’m over 60?
New registrants over 60 years old are generally not qualified for SSS membership, except for surviving spouse pensioners or guardians of a pensioner. If you are under 60 and not yet an SSS member, register as soon as possible — the earlier you start contributing, the higher your future benefits.
I already have an SS Number from a previous employer. Do I need to fill out an E1?
No — your SS Number is a lifetime unique number. You never apply for a new one. If you are changing membership type (e.g., from employed to self-employed), submit a Member Data Change Request Form (SSS Form E-4) instead. If you forgot your SS Number, recover it at my.sss.gov.ph using your UMID or personal information.
What does Temporary vs Permanent membership status mean?
When your SS Number is first generated online, your membership is tagged as Temporary. A Temporary SS Number can only be used for contribution payments. To claim any SSS benefit or apply for a loan, your status must be Permanent — which requires submitting your physical documents to an SSS branch. Always complete this step promptly after registering online.
