Where to Get the Forms

PSSLAI publishes its member forms in one place: the Downloadable Forms page inside Members Corner on its official site. Every form listed below is downloaded from there.

If you would rather not download and print, the same forms are available at a PSSLAI office, and PSSLAI’s head office is at 524 EDSA, Cubao, Quezon City 1109.

Documents You Need

What you bring depends on which branch of the service you are from and which product you are applying for.

  • Salary loan — PNP: your PNP ID, or any government-issued valid ID.
  • Salary loan — BFP: your BFP ID, one valid ID, a recent duty status, and a recent certificate of non-pending case.
  • Back-to-back loan: an updated PSSLAI ID, plus the PSSLAI account being pledged as collateral.
  • Vehicle loan — brand new: an updated PSSLAI ID and a quotation from the dealer.
  • Vehicle loan — pre-owned: an updated PSSLAI ID and either a dealer quotation or a Deed of Absolute Sale from an individual seller.

Deadlines and How Long It Takes

Two PSSLAI forms are genuinely deadline-driven, and missing the cut-off means waiting a full cycle:

  • Advances to Members Authorization (AMA): to receive your cash advance for July, the form must be submitted and validated by authorised PSSLAI personnel on or before 30 June of that year. If you have submitted an AMA form before, you do not need to submit again — PSSLAI honours the one already on file.
  • Bank Crediting Enrollment: enrolment cut-offs are 30 June for the first semester and 29 December for the second.

On the loan side the picture is the opposite of slow. PSSLAI states that requirements are minimal and that loan proceeds can be released in as fast as one hour. Calamity loans carry their own window — an affected member must avail within ninety days of the calamity declaration.

If Something Goes Wrong

Lost your PSSLAI ID or passbook? You need the Member Update Request Form plus an affidavit of loss, which costs ₱100; a passbook replacement is ₱50 and a new ID and passbook set is ₱140.

Changed your mobile number and locked yourself out of iTrack? That is also the Member Update Request Form — it covers reactivation, update and change of the mobile number registered to the facility.

If you have a complaint PSSLAI cannot resolve, it is regulated by the Bangko Sentral ng Pilipinas, which can be reached on (02) 8 708-7087 or at consumeraffairs@bsp.gov.ph.

Frequently Asked Questions

Where do I download the PSSLAI loan application form?

There isn’t one. This is the single most common misunderstanding about PSSLAI forms: its Downloadable Forms page carries membership and account forms only — application, member update, endorsement, termination, account update, AMA, bank crediting, death benefit and scholarship. There is no downloadable loan application form. Loans are applied for through PSSLAI directly, via a branch, the mobile app, or the Inquire Now route on its loan pages, and the documents you need are listed above.

How do I withdraw my capital contribution?

Withdrawing your CapCon entirely means ending your membership, which is the Membership Termination Form. If what you actually want is your CapCon net income paid out rather than held, that is the Bank Crediting Enrollment Form, with cut-offs of 30 June and 29 December.

How do I add my spouse or child as a member?

Use the Certificate of Endorsement for PSSLAI Membership Form, which is how a Regular Member endorses a legal spouse and children for Associate Membership.

Do I need to submit the AMA form every year?

No. PSSLAI states that members who previously submitted an AMA form are not required to submit again, because the association will honour the form already received.
