How PhilHealth Contribution is Computed

Your PhilHealth premium is 5% of your monthly basic salary — but it only applies to the portion of your salary between the income floor and the income ceiling. If you earn less than ₱10,000, your premium is still computed on ₱10,000, giving a minimum total premium of ₱500. If you earn more than ₱100,000, your premium is capped at ₱5,000 regardless of how high your salary goes. For all salaries in between, the formula is straightforward: salary multiplied by 5%.

For employed members, the 5% is split equally between you and your employer. You pay 2.5% and your employer pays 2.5%. Only your 2.5% share appears as a deduction on your payslip — your employer’s share is paid separately and never reduces your take-home pay. This equal split has applied since the premium reached its final 5% rate under PhilHealth Circular No. 2019-0009.

What Your PhilHealth Premium Covers

Every peso of your PhilHealth premium goes into the National Health Insurance Fund, which pays for your benefits when you need medical care. As an active contributing member you are entitled to inpatient hospital coverage, surgical procedures, maternity benefits, outpatient consultations under the Konsulta package, and selected diagnostic tests. Coverage extends automatically to your legal dependents — your spouse, unmarried children under 21, and parents over 60 — at no additional premium cost.

The No Balance Billing policy is one of the most important protections PhilHealth provides. Under this policy, indigent members and senior citizens admitted to ward accommodation at government hospitals cannot be charged anything beyond what PhilHealth covers. For other members, PhilHealth coverage significantly reduces out-of-pocket expenses — though private hospitals may charge the difference between the actual bill and the PhilHealth benefit package. Always check whether your hospital and doctor are PhilHealth-accredited before admission to maximize your coverage.

Frequently Asked Questions

Is PhilHealth contribution based on gross or basic salary?

Basic salary only. Allowances, overtime pay, bonuses and other variable compensation are not included in the computation base. If your payslip shows a basic salary of ₱25,000 plus a transportation allowance of ₱3,000, your PhilHealth premium is computed on ₱25,000 only — giving a monthly deduction of ₱625.

What if my employer is not remitting my PhilHealth contributions?

You can check your posted contributions through the PhilHealth Member Portal at philhealth.gov.ph. If your employer is deducting PhilHealth from your payslip but not remitting it to PhilHealth, this is a legal violation. You can file a complaint directly with PhilHealth — your benefits eligibility should not be affected since the liability falls on the employer, not on you.

Do voluntary and self-employed members pay the same rate?

Yes — the 5% rate applies to all member types. The difference is that voluntary and self-employed members shoulder the full 5% themselves since there is no employer to split it with. They also choose their own monthly income declaration, subject to the same ₱10,000 floor and ₱100,000 ceiling. Payment can be made monthly, quarterly, semi-annually, or annually.

Will the PhilHealth rate increase again?

The 5% rate is the final scheduled rate under the Universal Health Care Act (RA 11223). PhilHealth has confirmed no further increase. Any change beyond 5% would require a new law or amendment — there is no automatic scheduled increase beyond this level.
