Why the Government Cares About Your Health Insurance

Australia runs three financial nudges to push higher earners toward private hospital cover. The Medicare Levy Surcharge is the stick: earn over $105,000 (singles, 2026–27) or $210,000 (families) without hospital cover, and you pay an extra 1–1.5% of your income in tax — on top of the normal 2% Medicare levy. For many people above the threshold, the surcharge costs more than a basic hospital policy.

The private health insurance rebate is the carrot: the government pays up to 24.118% of your premiums (more if you’re 65+), shrinking as your income climbs through the same tiers and vanishing at Tier 3. And Lifetime Health Cover loading is the clock: wait past the 1 July after your 31st birthday to buy hospital cover, and every year of delay adds 2% to your premiums (up to 70%) — a loading that only clears after 10 continuous years of cover. Income here means your income for MLS purposes — broadly taxable income plus reportable fringe benefits, reportable super contributions and net investment losses, so it’s usually a little higher than your taxable income. Family thresholds rise by $1,500 for each dependent child after the first. Rebate percentages adjust every 1 April; the tier thresholds move each 1 July.
